Office Hours

Office hours are Monday through Friday from 8:00 a.m. to 5:00 p.m.

Fees

Co-pay and deductible payments for services are due at the time of your visit. If you have any questions regarding our fees, our staff is available to assist you with financial arrangements. For your convenience, we accept credit and debit cards. We can assist you as well if you do not have insurance coverage and need financial arrangements for services provided.

Appointments

Call 256-737-5115 To ensure that we are able to give you the best time appointment for your needs please call as soon as possible. If you are unable to make an appointment, or need to reschedule, please notify us at least 24 hours in advance.

Emergencies: If you need to consult with a doctor after regular office hours, you may call the Cullman Regional Medical Center operator at (256) 737-2000. The operator will assist you in reaching a doctor.

Insurance

As a service to you, our office will prepare and submit all insurance claims for you. However, we do consider the patient primarily responsible for the account. Please present your insurance identification card at the time of your visit. You will also be ask to present you driver's license at the time of the visit for additional information for verification.

Medicare

We are participating Medicare physicians. All claims will be filed for you. You will be responsible for your annual deductible, co-pay (if applicable) and the 20% Medicare does not pay. If you have Medicare supplement insurance, we will file a claim to them for the 20% balance as a courtesy to you.

Other

We also accept most all other Insurance plans. All claims will be filed for you. You will be responsible for your annual deductible, co-pay (if applicable) and any remaining percentage that the insurance does not pay that is the patient's responsibility. We ask that you pay your responsibility amounts at the time of service.

No Insurance

If you do not have insurance you will need to make financial arrangements with our staff prior to services being provided.
